Case Management
BATF provides three levels of case management services.


COBRA
Comprehensive case management model to serve persons and their families living with HIV/AIDS with multiple complex psychosocial and/or health related needs and their families.

Eligibility Requirements
Individuals and families infected by HIV/AIDS and are eligible for Medicaid.

MHRA
Comprehensive case management model to serve persons living with HIV/AIDS with multiple complex psychosocial and/or health related needs and their families Eligibility Requirements Individuals and families infected by HIV/AIDS, medical insurance is not needed.

Eligibility Requirements
Individuals and families infected by HIV/AIDS, medical insurance is not needed.

AI
Family centered low-intensive case management services, targeting HIV infected and affected individuals and families, as well as individuals whose behaviors place them at high risk for HIV/AIDS.

Eligibility Requirements
Individuals and families infected/affected by and/or those at high risk for HIV/AIDS. No medical insurance is needed.

Managed Care Services






Services Provides orientation to managed care, conducts health risk assessments and case finding for case management program.

Eligibility Requirements
Referred by Metro Plus or VIDA Care.
